Dominion Energy (North Anna Power Station) (EA-24-126)
On December 11, 2024, the NRC issued a notice of violation to Dominion Energy (licensee) associated with a white significance determination process finding at North Anna Power Station. The white finding, an issue of low-to-moderate safety significance, involved the licensee’s failure to implement adequate procedural instructions to control foreign material during the assembly and installation of a control circuit relay, which resulted in the inoperability of an emergency diesel generator. The failure to implement adequate instructions was contrary to Title 10 of the Code of Federal Regulations, Part 50, Appendix B, Criterion V, "Instructions, Procedures, and Drawings."
Tennessee Valley Authority (Browns Ferry) (EA-24-075)
On November 18, 2024, the NRC issued a notice of violation to Tennessee Valley Authority (licensee) associated with a white significance determination process finding at Browns Ferry Nuclear Plant. The white finding, an issue of low-to-moderate safety significance, involved the licensee’s failure to identify and correct a degraded rupture disc, which resulted in the inoperability of the high pressure coolant injection system. The failure to promptly identify and correct the deficiency was contrary to Title 10 of the Code of Federal Regulations, Part 50, Appendix B, Criterion XVI, "Corrective Actions."
Constellation Energy Generation, LLC (James A. Fitzpatrick Nuclear Power Plant) (EA-24-088)
On October 29, 2024, the NRC issued a notice of violation associated with a white significance determination process finding to Constellation Energy Generation, LLC (licensee) at James A. Fitzpatrick Nuclear Power Plant. The white finding, an issue of low-to-moderate safety significance, involved the licensee’s failure to provide adequate qualitative or quantitative acceptance criteria in work instructions during maintenance, which resulted in the inoperability of the 'B' emergency diesel generator, contrary to Title 10 of the Code of Federal Regulations, Part 50, Appendix B, Criterion V, “Instructions, Procedures, and Drawings,” and Technical Specifications.
Duke Energy Carolinas, LLC (Catawba Nuclear Station) (EA-24-049)
On September 5, 2024, the NRC issued a notice of violation associated with a white significance determination process finding to Duke Energy Carolinas, LLC at Catawba Nuclear Station. The white finding, an issue of low-to-moderate safety significance, involved the licensee’s failure to maintain the functionality of the emergency ventilation system, which resulted in the inoperability of the 2A emergency diesel generator, contrary to Title 10 of the Code of Federal Regulations, Part 50, Appendix B, Criterion III, “Design Control,” and Technical Specifications.
Southern Nuclear Operating Company, Inc. (EA-23-139)
On August 27 2024, the NRC issued a notice of violation to Southern Nuclear Operating Company, Inc (licensee) for a Severity Level III problem involving two violations at the Edwin I. Hatch Nuclear Plant (Hatch), Units 1 and 2. The violations involved the licensee’s failure to: 1) maintain records of radiation and contamination surveys that were complete and accurate in all material aspects as required by Title 10 of the Code of Federal Regulations (10 CFR) 50.9(a), and 2) make or cause to be made surveys of radiologically controlled areas that were reasonable under the circumstances to evaluate radiation levels, concentrations or quantities of residual radioactivity, and the potential hazards associated with radiation levels or residual radioactivity as required by 10 CFR 20.1501(a)(2). These violations were the result of contamination and radiation survey records that were falsified by a Hatch senior radiation protection technician. These records are necessary to establish adequate protective measures related to radiological conditions and hazards to workers at the plant.
Cooper Nuclear Station (EA-24-001)
On April 1, 2024, the NRC issued a notice of violation to Nebraska Public Power District (licensee) for a Severity Level (SL) III violation at Cooper Nuclear Station. The SL III violation involved the licensee's failure to notify the Region IV Regional Administrator within 30 days of learning of a change in the medical condition of two licensed operators as required by Title 10 of the Code of Federal Regulations 50.74.
