2025 Individual Actions

Mr. Joel Neff (EAI-RI-2025-0005)

On May 19, 2025, the U.S. Nuclear Regulatory Commission issued a notice of violation to Mr. Joel Neff for an SL III violation of Title 10 of the Code of Federal Regulations 50.5, “Deliberate misconduct.”

Mr. Valmicky Samial (EAI-RI-2025-0006)

On May 19, 2025, the U.S. Nuclear Regulatory Commission issued a notice of violation to Mr. Valmicky Samial for an SL III violation of Title 10 of the Code of Federal Regulations 50.5, “Deliberate misconduct.”

Mr. Keith Brockman (IA-24-019/EAI-RIV-2025-0010)

On March 13, 2025, the NRC issued a notice of violation to Mr. Keith Brockman for a Severity Level III violation. Mr. Brockman, a former Reactor Operator for Arkansas Nuclear One (ANO) engaged in deliberate misconduct, a violation of Title 10 of the Code of Federal Regulations 50.5(a)(1). Specifically, he deliberately failed to adhere to an ANO operating procedure by accessing non-work-related internet sites hundreds of times without authorization while on watch.

Mr. Mohammad Aljallad (IA-24-021)

On March 12, 2025, the NRC issued a notice of violation to Mr. Mohammad Aljallad for a Severity Level III violation. Mr. Aljallad, a former Radiation Safety Officer for Hartford Hospital (Hartford) engaged in deliberate misconduct, a violation of Title 10 of the Code of Federal Regulations 30.10(a)(2). Specifically, he deliberately prepared and submitted a Hartford contamination spill report in which he documented inaccurate contamination survey results.

Mr. John Mann (IA-24-023)

On January 31, 2025, the NRC issued a notice of violation to Mr. John Mann for a Severity Level III violation. Mr. Mann, a licensed reactor operator assigned to the Oconee Nuclear Station, violated the terms and conditions of his license when he performed activities authorized by his license while under the influence of alcohol that could adversely affect his ability to perform licensed duties safely and competently, as required by Title 10 of the Code of Federal Regulations 55.53(j).
