Rio Piedras Sono-Nuclear Center - Puerto Rico (EAF-Rl-2025-0151)
On February 10, 2026, the NRC issued a notice of violation and proposed imposition of civil penalty in the amount of $9,000 to Rio Piedras Sono-Nuclear Center, for a severity level III violation of Title 10 of the Code of Federal regulations 20.2001 “General requirements”.
Construction Testing Services, INC. (EAO-RIV-2025-0167)
On January 21, 2026, the U.S. Nuclear Regulatory Commission (NRC) issued a Notice of Violation (NOV) to Construction Testing Services, INC. The NOV identified one Severity Level III violation for failing to file for reciprocity with the NRC for calendar year 2025 before performing work in an area of exclusive Federal jurisdiction at Travis Air Force Base as required by Title 10 of the Code of Federal Regulations (10 CFR) Part 150.20(b)(1).
Centerpoint Medical Center of Independence, LLC (EAF-RIII‑2025‑0164)
On January 21, 2026, the NRC issued a notice of violation to Centerpoint Medical Center of Independence, LLC, for a severity level III problem associated with violations of Title 10 of the Code of Federal regulations 20.1502(a)(1) “Conditions requiring individual monitoring of external and internal occupational dose”; and License Condition No.13.A of NRC License 24‑18655-01.
Valley Health Winchester Medical Center (EAF-RI-2025-0168)
On January 20, 2026, the NRC issued a notice of violation to Valley Health Winchester Medical Center for a severity level III violation for failing to seek required NRC approval before naming a RSO on the licensee and to have a RSO in place for approximately 7 months. as required by 10 Code of Federal Regulations 35.13(c) and 35.24(b), respectively.
Mercy Hospital - St Louis (EAF-RIII-2025-0170)
On January 14, 2026, the U.S. Nuclear Regulatory Commission (NRC) issued a Notice of Violation (NOV) to Mercy Hospital (licensee). The NOV identified one Severity Level III violation for the licensee’s failure to prepare written directives that were dated and signed by an authorized user (AU) before multiple administrations of iodine-131 sodium iodide greater than 1.11 megabecquerels (30 microcuries (μCi)), as required by Title 10 of the Code of Federal Regulations (10 CFR) Part 35.40(a).
