2024 Individual Actions

Mr. David Huey (IA-24-007)

On December 12, 2024, the NRC issued an Order prohibiting involvement in NRC-Licensed activities for three years to Mr. David Huey, a manager for Titan Inc. (Titan), for deliberately directing a radiographer to perform radiography without a second qualified individual being present. The NRC determined that the manager’s actions violated Title 10 of the Code of Federal Regulations (10 CFR) 30.10(a)(1), which caused Titan to be in violation of 10 CFR 34.41(a).

Mr. James Bradshaw (IA-2024-009)

On October 7, 2024, the NRC issued a notice of violation to Mr. James Bradshaw for an SL III violation. Mr. Bradshaw, a licensed reactor operator assigned to South Texas Project Electric Generating Station, deliberately submitted to the facility licensee medical information he knew to be incomplete and inaccurate, contrary to requirements in 10 CFR 50.5(a)(2). Additionally, he was also issued an SL IV violation for failure to comply with a condition imposed by the Commission on his license regarding his failure to take prescribed medication resulting in a violation of 10 CFR 55.53(I).

Mr. Robert Gould (IA-24-016)

On August 30, 2024, the NRC issued a notice of violation to Mr. Robert Gould for an SL IV violation. Mr. Gould, a licensed reactor operator assigned to Donald C. Cook Nuclear Plant, failed to complete a tour of the plant and to review all shift turnover procedures before resuming licensed activities resulting in a violation of 10 CFR 55.53(e).

Mr. Eric Beck (IA-24-015)

On August 30, 2024, the NRC issued a notice of violation to Mr. Eric Beck for an SL IV violation. Mr. Beck, a licensed reactor operator assigned to Donald C. Cook Nuclear Plant, failed to complete a tour of the plant and to review all shift turnover procedures before resuming licensed activities which resulted in a violation of 10 CFR 55.53(e).

Mr. Jonathan Burke (IA-24-014)

On August 30, 2024, the NRC issued a notice of violation to Mr. Jonathan Burke for an SL IV violation. Mr. Burke, a licensed reactor operator assigned to Donald C. Cook Nuclear Plant, failed to complete a tour of the plant and to review all shift turnover procedures before resuming licensed activities which resulted in a violation of 10 CFR 55.53(e).

Mr. Timothy Schuck (IA-24-013)

On August 30, 2024, the NRC issued a notice of violation to Mr. Timothy Schuck for an SL IV violation. Mr. Schuck, a licensed reactor operator assigned to Donald C. Cook Nuclear Plant, failed to complete a tour of the plant and to review all shift turnover procedures before resuming licensed activities which resulted in a violation of 10 CFR 55.53(e).

Mr. Steven Vonk (IA-24-012)

On August 30, 2024, the NRC issued a notice of violation to Mr. Steven Vonk for an SL IV violation. Mr. Vonk, a licensed reactor operator assigned to Donald C. Cook Nuclear Plant, failed to complete a tour of the plant and to review all shift turnover procedures before resuming licensed activities which resulted in a violation of 10 CFR 55.53(e).

Mr. Lyle Baker (IA-24-011)

On August 30, 2024, the NRC issued a notice of violation to Mr. Lyle Baker for an SL IV violation. Mr. Baker, a licensed reactor operator assigned to Donald C. Cook Nuclear Plant, failed to complete a tour of the plant and to review all shift turnover procedures before resuming licensed activities which resulted in a violation of 10 CFR 55.53(e).

Mr. Eric Tulson (IA-24-010)

On August 30, 2024, the NRC issued a notice of violation to Mr. Eric Tulson for an SL IV violation. Mr. Tulson, a licensed reactor operator assigned to Donald C. Cook Nuclear Plant, failed to complete a tour of the plant and to review all shift turnover procedures before resuming licensed activities which resulted in a violation of 10 CFR 55.53(e).

Mr. Brian Luketich (IA-24-006)

On September 3, 2024, the NRC issued a notice of violation (Notice) to Mr. Luketich for a Severity Level III violation of 10 CFR 30.10(a) and 10 CFR 34.41 requirements. Mr. Luketich, a radiographer working for Titan Inspection, Inc., on November 16, 2022, willfully conducted radiography operations without being accompanied by at least one other qualified radiographer or an individual who at a minimum met the requirements of 10 CFR 34.43(c).

Mr. Travis Hodges (IA-24-001)

On August 27, 2024, the NRC issued a notice of violation to Mr. Hodges for a Severity Level III violation. Mr. Hodges, a senior radiation protection technician for Southern Nuclear Operating Company’s Edwin I. Hatch Nuclear Plant (Hatch) engaged in deliberate misconduct, a violation of Title 10 of the Code of Federal Regulations 50.5, “Deliberate Misconduct,” when he deliberately submitted falsified radiation and contamination surveys to Hatch. Specifically, on multiple occasions, Mr. Hodges presented survey results as if he had obtained the data from actual physical surveys of plant areas, when in fact he did not physically enter the areas to conduct the surveys but instead copied the data from previously completed surveys.

Martin Ramos (IA-23-009​)

On May 22, 2024, the NRC issued a letter to Mr. Martin Ramos, a former Mistras employee, stating he willfully falsified calibration certificates for non-destructive test equipment that was used at nuclear power plants. After considering a criminal conviction which includes a 2-year ban from participating or being employed in NRC licensed activities, the NRC has concluded no further action is warranted on this matter.

Miguel Marcial Amaro (IA-23-004)

On May 22, 2024, the NRC issued a letter to Mr. Miguel Marcial Amaro, a former Mistras employee, stating he willfully falsified calibration certificates for non-destructive test equipment that was used at nuclear power plants. After considering a criminal conviction which includes a 5-year ban from participating or being employed in NRC licensed activities, the NRC has concluded no further action is warranted on this matter.


Mr. Keith Miller (IA-24-005)

On April 2, 2024, the NRC issued a notice of violation to Mr. Keith Miller for an SL III violation. Mr. Miller, a licensed reactor operator assigned to James A. FitzPatrick Nuclear Power Plant, violated the terms and conditions of his license when he failed to participate in the James A. FitzPatrick Nuclear Power Plant drug and alcohol testing program by refusing to provide a specimen for testing when selected for a random test, as required by 10 CFR 55.53(k).
