|Mr. Pieter Odendaal
On February 23, 2021, the U.S. Nuclear Regulatory Commission issued a severity level III notice of violation to Mr. Pieter Odendaal for a violation of Title 10 of the Code of Federal Regulations 50.5(a)(2) when he deliberately submitted to the licensee information that he knew to be incomplete or inaccurate concerning (1) a trainee's qualification to conduct fuel handling activities, and (2) the submission of a written statement that omitted the material fact that the unqualified trainee operated the refueling machine during a fuel handling incident.
|Mr. Maurice Omaits
||On December 15, 2021, the NRC issued a Notice of Violation to Mr. Maurice Omaits, a senior engineering training instructor employed by Entergy Operations, Inc. (licensee) at the Grand Gulf Nuclear Station, for willfully compromising engineering support qualification exams by providing inappropriate assistance to students in violation of 10 CFR 50.5, "Deliberate misconduct." This is a Severity Level III violation in accordance with NRC Enforcement Policy Section 2.2.1.d.
