Information Notice No. 89-89: EVENT NOTIFICATION WORKSHEETS
Addressees:
All holders of operating licenses or construction permits for nuclear power
reactors.
Purpose:
This information notice is intended to alert addressees to a recent revision
to the NRC Operations Center's "Event Notification Worksheet" and delineates
the event notification information that is sought by the NRC Operations
Officers. However, suggestions contained in this information notice do not
constitute NRC requirements; therefore, no specific action or written
response is required.
Discussion:
The NRC "Event Notification Worksheet," NRC Form 361, has been revised to
assist the NRC Headquarters Operations Officers in obtaining adequate infor-
mation for evaluation of significant events reported to the NRC Operations
Center. The new forms more accurately reflect the event classifications and
the 10 CFR 50.72 categories that must be reported. A copy of the new work-
sheet is enclosed for your reference. NRC Form 361 can be ordered from the
NRC Information and Records Management Branch at (301) 492-8138.
The blocks for the "Cause of Failure," "System," and "Component" have been
deleted, and this information is now included under the description section
where it can be more fully described. As in the past, the description is to
provide adequate detail so that the Operations Officer will be able to
understand the event and its significance. It is recognized that all the
information regarding an event may not be available at the time of
notification. However, reports are to be as clear and complete as possible
within the time allowed by the immediate notification requirements. If
significant information not included in the original report is identified,
original information requires correction, or the classification of an event
is changed, this information can be reported in an update notification to
the Operations Center when it becomes available.
The back of the form, which is used to collect information on radiological
releases and/or reactor coolant system (RCS) or steam generator (SG) tube
leaks, has been revised to provide a simpler and more structured method of
obtaining and evaluating information pertinent to these types of events.

Addressees may wish to make copies of the new worksheet available to
employees responsible for reporting event-related information to the NRC to
replace copies of the worksheets provided in Information Notice No. 85-78,
"Event Notification," dated September 23, 1985.
These worksheets or similar event write-ups can be sent to the NRC
Operations Center by facsimile as an aid in the notification process, if so
desired. However, neither the use of NRC Form 361 nor the use of facsimile
transmittals is required and in fact is not desired if it would delay the
required telephone notifications to the NRC Operations Center. The NRC
Operations Center's facsimile number is (301) 492-8187.
This information notice requires no specific action or written response. If
