2012 Inspection Reports

The purpose of inspection reports is to document the inspection scope, observation, and findings of inspections conducted by the NRC. The NRC performs inspections to oversee the commercial nuclear industry to determine whether its requirements are being met by licensees and their contractors. The NRC performs inspections to verify the quality and suitability of vendor products, licensee-vendor interface, and review of equipment problems found during operation and their corrective action.

Prior to December 1999, the NRC issued NUREG-0040, "Licensee Contractor and Vendor Inspection Status Report," (The White Book) to document the results of inspections performed by the NRC's Quality Assurance, Vendor Inspection, Maintenance and Allegations Branch that were distributed to the inspected organizations. As of November 1999, these inspection reports are available electronically in the NRC's record retrieval system, ADAMS. The following inspections have been conducted since May 2005:

Mitsubishi Heavy Industries, Ltd. (MHI), Kobe, Japan, limited scope inspection to assess MHI's compliance with selected portions of their quality assurance (QA) and 10 CFR Part 21 programs specifically, the quality assurance program will be reviewed to determine if the preparation and testing of the Full-Scale Mock-up and testing of re-designed anti-vibration bars are adequate and correct and meet the applicable requirements of Appendix B to 10 CFR Part 50, 10 CFR Part 21, and American Society of Mechanical Engineers Code Section III, "Rules for Construction of Nuclear Facility Components."

Westinghouse Electric Company (WEC), Cranberry Township, PA., limited scope inspection to assess WEC's compliance with selected portions of their quality assurance (QA) and 10 CFR Part 21 programs specifically related to corrective actions to issues identified during two previous NRC inspections: (1) the Engineering Design Verification Inspection (Inspection Report No. 99900404/2011-201) performed in June and July, 2011; and (2) the Environmental Qualification Inspection (Inspection Report No. 99900404/2012-201) performed in March of 2012.

Shaw Modular Solutions (SMS), Charles, LA, limited-scope inspection to assess SMS's compliance with selected portions of their quality assurance (QA) and 10 CFR Part 21 programs. Specifically, this inspection evaluated SMS's implementation of quality activities associated with the fabrication of structural modules for V.C. Summer Units 2 and 3 and Vogtle Units 3 and 4.

National Technical Systems (NTS) facility in Acton, MA, limited-scope routine inspection to assess NTS's compliance with selected portions of their quality assurance (QA) and 10 CFR Part 21 programs specifically associated with the seismic qualification testing of the tension bolt and position switch of the AP1000 design squib valves.

(NLI) facility in Fort Worth, TX, and at the Nemko Dallas USA, Inc. facility in Lewisville, TX, limited-scope inspection to assess NLI's compliance with selected portions of their quality assurance (QA) program and conformance to Regulatory Guide 1.180, "Guidelines for Evaluating Electromagnetic and Radio-Frequency Interference in Safety-Related Instrumentation and Control Systems," Revision 1, issued October 2003, as committed to in the AP1000 Design Control Document, Tier 2, Revision 19.

Kinectrics facility in Toronto, Ontario, limited-scope routine inspection to review implementation of Kinectric's quality assurance (QA) program and 10 CFR Part 21 program, specifically as it pertains to Kinectrics supply of testing services to support environmental qualification of components being supplied as part of the Westinghouse AP1000 reactor design as well as reviewing selected activities in support of testing and dedication of safety related components to the U.S. nuclear operating fleet.

Fauske & Associates, LLC (FAI), Burr Ridge, IL., limited-scope routine inspection was to assess FAI's compliance with quality assurance (QA) program and 10 CFR Part 21 program, specifically associated with QA activities associated with the environmental qualification type testing of instrument cable in support of AP1000 licensing activities, specifically the postulated loss-of-coolant accident design-basis event. These tests, including qualification and functional tests are associated with or directly impact closure of inspections, tests, analyses, and acceptance criteria (ITAAC) from Revision 19 of the certified AP1000 design. Currently, these ITAAC are incorporated into the combined licenses of Vogtle Units 3 and 4 and V.C. Summer Units 2 and 3.
