Below is a description of the incident, as described in a DOD record titled, Narrative Summaries of Accidents Involving U.S. Nuclear Weapons, 1950-1980: Februarv 13, 1950 / B-36 / Pacific Ocean, off Coast of British Columbia The B-36 was en route from Eielson AFB to Carswell AFB on a simulated combat profile mission. The weapon aboard the aircraft had a dummy capsule installed. After six hours of flight, the aircraft developed serious mechanical difficulties, making it necessary to shut down three engines. The aircraft was at 12,000 feet altitude. Icing conditions complicated the emergency and level flight could not be maintained. The aircraft headed out over the Pacific Ocean and dropped the weapon from 8,000 feet. A bright flash occurred on impact, followed by a sound and shock wave. Only the weapon's high explosive material detonated. The aircraft was then flown over Princess Royal Island where the crew bailed out. The aircraft wreckage was later found on Vancouver Island. |

This refers to orphan source incidents investigated in which the radioactive source was ultimately not located. This request includes all incidents in which an investigation was initiated into a specific orphan source, which ultimately did not produce a culprit due to factors such as the source being lost somewhere over a large area (such as the caesium-137 capsule lost but ultimately found in Australia 2023), issues with retrieval such as the loss of the object into a body of water or underground location, or any other factors which prevented the recovery of the object. In addition to the Australian incident, a similar but unrelated occurrence would be the 1958 Tybee Island incident, which left radioactive materials in the water off the coast of Georgia, but made these materials unrecoverable due to the size of the search area. The Tybee incident is used only to demonstrate an example of radioactive materials being deemed unrecoverable or lost. While the Tybee Island incident would be classified as a "broken arrow", this request refers only to incidents deemed by the NRC to be "orphan sources". The full list should include dates, locations, descriptions of materials lost, a summary of circumstances causing the incident, and reasons in which retrieval was not possible |

To Whom It May Concern, I would like to access the undisclosed public comments submitted as part of the NRC’s regulatory proposal in the docket number NRC-2015-0070-0229. As part of the proposed rule, ‘Regulatory Improvements for Production and Utilization Facilities Transitioning to Decommissioning’, the NRC invited public comments in 2015. According to the docket information on regulations.gov, there were 4,347 public comments received. However, only 153 were made public on regulations.gov. That means that 4,194 public comments are missing. I understand that there are often good reasons for withholding some comments (e.g., duplicate comments, silly comments, obscene comments etc). However, I would like to see the original missing comments to understand better what those comments contained. I am a U.S. citizen but I currently work at Leiden University in the Netherlands where I am carrying out research on the regulatory public comments process in the US federal government. I would be grateful if you could share the public comments with me in an electronic file such as a PDF or an excel document. If preferable, I can also receive hard copies of the files to my address in the United States that I include with this FOI request. I do not need to receive all 4,347 of the comments but just the missing 4,194. If is not possible to separate the 4,194 from the total then I would also happily receive the total number of comments. |
