Instructions for completing this form are provided on Pages 1-2 of the form.

Completed Unclassified NRC Form 790s shall be submitted in the following manner:

• By mail addressed to: ATTN: Document Control Desk, Director, Division of Security Operations, Office of Nuclear Security and Incident Response, U.S Nuclear Regulatory Commission, Washington, DC 20555–0001;
• By hand delivery to the NRC's offices at 11555 Rockville Pike, Rockville, Maryland; or
• Where practicable, by electronic submission, to ISBRCA.Resource@nrc.gov.

Completed Classified NRC Form 790s shall be transmitted in accordance with 10 CFR 95.39 to the NRC Headquarters' classified mailing address listed in Appendix A of 10 CFR Part 73.
