This letter is a notification of a deviation concerning our Class 1E type CV-2 relays.

We have evaluated six CV-2 relays returned to us by Duke Energy Corporation and found the relays were missing a weight assembly that is used in balancing the moving disc. The weight assembly corrects timing inconsistencies which may occur during a seismic event.

These six relays had identifying marks on the disc and element assemblies which we concluded were made by two temporary sub-assemblers who we no longer employ. There are two other customers who we have sold CV-2 relays to (quantity two each) during the time in which these assemblers were employed, they are, Korea Hydro & Nuclear Power Co. Ltd. and Wesco in Aurora, Colorado. These customers will be promptly notified of this potential deviation, and if affected, to return their relays to us for the addition of the weight assembly and recertification.

Duke Energy Corporation has indicated to us they plan to return the remainder (twenty) of their CV-2 relays that are missing the weight assembly by the end of the first week of July, 2002. If investigation of those relays conclude further notification is required, we will do so at that time.

We have since taken internal corrective action, including the addition of a final inspection verification to assure this deviation does not occur again.
