Subject: Trouble Retracting Source
This is a 30-day notification in accordance with 10CFR Part 34.
On April 5, 1995, difficulty was encountered in retracting the cobalt 60
source in the north exposure room into its Amersham 680 projector, serial
no. 381. It was finally retracted to the safe storage position after
repeated cranking in and out. This is the source mentioned in my letter
of March 24, 1995, that has been operated manually pending repair and
return of the 957 automatic exposure controller. There was no apparent
damage nor loose connections in the guide tube; however, it appeared as
though there might be a misalignment between the S-tube and the guide
tube connection nut. Nothing else was noted in the examination. The unit
was taken out of service after the examination. It should be noted that
this 680 has been in constant service since January, 1991, without any
problems. The only modification has been the installation of locking
device required by 10 CFR Part 34.
Amersham was notified on the incident and recommended that we ship them
the 680 for examination and testing. The unit will be shipped after
receipt of shipping labels from Amersham. Amersham stated that they would
perform the 10CFR Part 21 evaluation and make any required notification
to the NRC.
