The following information was provided by the South Carolina Department of Environmental Services (the Department) via phone and email:

"The licensee informed the Department on January 3, 2025, a portable moisture density device was hit by a piece of construction equipment which caused the device to be disabled or fail to function as designed. The portable moisture density device is a Humboldt 5001 gauging device (serial number 3521) housing approximately 10 millicuries of cesium-137 and 40 millicuries of americium-241:beryllium. The licensee did not report any overexposures. The licensee did not call the Department on January 3, 2025. The Department was made aware of the event on January 6, 2025.
