AGREEMENT STATE REPORT - MISSING DEPLETED URANIUM SOURCE

The following information was provided by the Florida Bureau of Radiation Control (BRC) via email:

"BRC received notification of a missing 270-pound depleted uranium source on January 14, 2025. Baptist Hospital discovered the missing source during an inventory performed in October, 2024. The last documented inventory where this material was observed occurred in 1998.

"Estimated activity: 40mCi"

THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL

Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. AGREEMENT STATE REPORT - UNPLANNED CONTAMINATION DUE TO FIRE

The following information was provided by the Tennessee Division of Radiological Health via email:

"An unplanned fire occurred during the cutting of waste material (steam generators) in a restricted area. Memphis Fire Department used light water and foam to contain the fire. The material involved (plutonium) did exceed five times the annual limit on intake. Berms were established to contain any contaminated water. The building did have negative ventilation. Particulate filters showed background levels and air sampling showed no contamination outside. Also, there was no damage to the facility or injuries to personnel.

"Corrective actions or reports as well as additional information will be updated with a NMED report within 30 days."
