AGREEMENT STATE REPORT - FIRE AT FACILITY CONTAINING RADIOACTIVE GAUGES

The following report was received via telephone and email by the Illinois Emergency Management Agency [the Agency]:

"At approximately 1900 [CDT] on 9/10/23, a fire/explosion was reported at the Archer Daniels Midland facility located at 4666 East Farie Parkway in Decatur, IL. Information available at this time indicates a flammable hexane environment may have contributed to the incident. The Agency contacted the facility at approximately 2100 [CDT] on 9/10/23 to determine the status of the 23 radioactive gauges located within the building. The gauges contain a quantity of radioactive material that if impacted by the fire could present non-life threatening but elevated exposure rates to first responders. At 0945 [CDT] on 9/11/23, the Agency made contact with the Radiation Safety Officer at the facility. He stated the gauges are in an adjacent part of the facility (shared wall) which remains to be determined if impacted. It was noted that the area containing the gauges has an elevated hexane concentration. The Agency is actively coordinating with the facility's radiation safety staff and the Macon County Emergency Management Agency to provide information to first responders and coordinate a site visit to determine the status of the devices. If additional information regarding integrity of the devices is not available by midday, the Agency will coordinate a site visit. Reportedly, site access is still limited and information is still forthcoming to emergency managers.

"The 23 Texas Nuclear Model 5205 gauges range in activity from 50 to 100 mCi of Cs-137. Information available indicates 4 gauges contain 100 mCi of Cs-137 and 19 gauges contain 50 mCi of Cs-137. 8 additional gauges containing up to 500 mCi of Cs-137 are located in other onsite buildings, but are not anticipated to have been impacted. It is unclear if the Texas Nuclear Model 5205 gauges or their containers have been impacted by the fire/explosion at this time. On scene assessment is not immediately possible and may not be within the 24-hour reporting timeframe. The matter is being reported at this time and will be retracted if the gauges are found to be undamaged."

The following is a summary of an update received via email from the Illinois Emergency Management Agency:

The licensee's hazmat response staff was able to enter the building late yesterday (9/11/23) and indicated none of the 23 radioactive gauges appear impacted by the fire. The Illinois Emergency Management Agency (IEMA) nuclear safety staff still plan to perform confirmatory surveys and wipes once the building can be safely accessed. IEMA staff are working with the licensee and county Emergency Management Agency officials to coordinate that site visit. Based on the information available, IEMA staff do not have any radiation exposure or accountability concerns at this time. This incident will be kept open until Agency staff have conducted a site visit and confirmed the integrity of the 23 devices.

AGREEMENT STATE REPORT - FAILURE OF SOURCE TO RETRACT TO SAFE POSITION

The following was reported by the Illinois Emergency Management Agency (the Agency) via email:

"The Agency was contacted by IRISNDT, Inc. the morning of 9/12/23 to advise of a stuck radiography source and successful source retrieval that occurred the previous morning at the Phillips 66 Refinery in Wood River, IL. Reportedly, the guide tube came in contact with a hot pipe, partially melted, and prohibited the 46 Ci Ir-192 source from returning to the shielded position. The radiation safety officer reports no overexposures to workers or members of the public as a result of this incident (maximum 12 mrem occupational). The source was reportedly not damaged and the impacted guide tube has been taken out of service.

"Limited additional details are available at this time. Agency staff are conducting a reactionary inspection tomorrow, 9/13/23, to review dosimetry, gather incident details, perform a time-motion study and evaluate the adequacy of the licensee's actions."

The following information was provided by the licensee via email:

"On 09/17/2023 at 2218 [EDT], Operations identified that the bearing cooling (BC) tower basin was overflowing. Earlier in the day, the BC tower was isolated as part of a planned maintenance evolution and the overflow condition was due to isolation valve leak-by. At 2255, the leak-by was corrected and stopped the overflow. Approximately 75 gallons may have been discharged to the lake from the overflow.

"The BC water was sampled by Chemistry and all chemical parameters were within VP DES (Virginia Pollutant Discharge Elimination System) limits.

"At 1420 on 09/18/23, a 24-hour notification was made to the Virginia Department of Environmental Quality (DEQ) in accordance with the North Anna VPDES permit.

"This issue is being reported per 10CFR50.72 (b)(2)(xi) due to the notification of other government agency."

The NRC Resident Inspector was notified.

AGREEMENT STATE - LOST STATIC ELIMINATORS

The following information was provided by the Texas Department of State Health Services (the Department) via email:

"On July 13, 2023, the Department was contacted by the Biomeric's quality assurance scientist, who stated she could not find two generally licensed devices. The devices are P-2024-1000 static eliminators. The devices were used to remove static electricity on small plastic parts. The devices were discovered missing during an inventory of devices that were to be replaced. The company stated the devices did not pose a risk of exposure to any individual. The company searched for the devices, but was unable to locate them. The company has implemented a monthly inventory program to keep better track of the devices. The devices will be centrally located when in storage from now on."

"On September 13, 2023, the Department was notified by Braskem Inc. that during routine testing, the shutter on two Vega model SH-F2 gauges failed to close. Shutters in the open position is the normal operating position. Both gauges contain 200 millicuries of Cesium-137. There is no risk of additional radiation exposure to members of the general public or radiation workers due to this event. Additional information has been requested of the licensee. Additional information will be provided in accordance with SA-300."

AGREEMENT STATE REPORT - STOLEN MOISTURE DENSITY GAUGE

The following was reported by the Florida Bureau of Radiation Control (BRC) via email:

"[The branch administrator and vice president] of Ardaman & Associates, called the BRC at 1030 EDT to report a stolen moisture density gauge. Their technician was at a job site where he says there are two entrances, but not much security to check who comes and goes. Before doing his readings, he noticed a truck parked close by but did not make anything of it since there are other workers on site. After he was done he went to the trailer to get his reports and left the gauge outside. A few minutes later he says someone came to notify him they saw an individual from the truck take possession of the gauge and drive off the site. They called the police to report the stolen Troxler gauge, serial number 36530 model 3430. Police report number to follow."

AGREEMENT STATE REPORT - STOLEN MOISTURE DENSITY GAUGE

The following was reported by the California Department of Public Health, Radiologic Health Branch (RHB) via email:

"On 09/12/2023, RHB received a call from [the] Principal Engineer, and President of A3GEO, Inc., regarding the loss of their moisture density gauge. They stated that on 09/12/2023 at 1445 PDT, the radiation safety officer (RSO) of A3GEO parked his 2001 Dodge Dakota Pickup Truck, containing InstroTek Model 3500 Xplorer with serial number 503 moisture density gauge, at the corner of Ridge Road and Euclid Avenue in Berkeley, CA. The RSO went into the corner market, and as he was exiting the corner store, he saw a man driving away his truck. He tried to stop him by running behind the truck, but he was unable to stop him from driving off. He immediately reported the incident to the Berkeley Police Department. RHB advised the licensee to place an ad in a local Berkeley newspaper offering a reward for return of the stolen gauge."

The following information was provided by the licensee via email:
"On 9/18/2023 at 2007 CDT, Clinton reported to the Illinois Emergency Management Agency, National Response Center and DeWitt County a hazardous substance release of 1300 gallons of Sodium Bisulphite. The release was at the site's flume discharge building due to a crack on a fitting inside the building. This release did not exceed any NRC regulations or reporting criteria. This notification is being made solely as a four-hour, non-emergency notification for a Notification of Other Government Agency. This event is reportable in accordance with 10 CFR 50.72(b)(2)(xi).

"There was no impact on the health and safety of the public or plant personnel. The NRC Resident Inspector has been notified."

CONCURRENT REPORT - HIGH CHLORINE LEVEL IN OUTFALL POND

The following information was provided by the licensee via email, facsimile, and phone:

"The Ohio Environmental Protection Agency was notified at approximately 1402 EDT on 09-19-2023, that the [National Pollutant Database] limit for chlorine at Outfall 012 (X-2230M Holding Pond) of 0.019 mg/L was exceeded on 09-18-2023, with a result of 0.109 mg/L with the elevated level being confirmed by a sample on the morning of 09-19-2023.

"Centrus American Centrifuge Operating is currently investigating to determine the cause of the exceedance."

NRC Region 2 was notified.
