- 10CFR PART 21 REPORT -

TWELVE SAFETY-RELATED 480-VOLT WESTINGHOUSE MODEL #DB-50 CIRCUIT BREAKERS WERE SENT TO WESTINGHOUSE REPAIR AND REPLACEMENT SERVICES, CHESWICK, PA, AN APPROVED 10CFR50, APPENDIX B QUALIFIED REPAIR FACILITY, FOR OVERHAUL.

DURING PRE-INSTALLATION TESTING, THE LICENSEE DETERMINED THAT LOW TRIP BAR FORCES AND THE WINDING OF THE TRIP PAN SPRING CONTRIBUTED TO BREAKER MIS-OPERATION. IF INSTALLED, THIS MIS-OPERATION MIGHT HAVE RESULTED IN THE BREAKERS MALFUNCTIONING WHEN CALLED UPON TO PERFORM THEIR SAFETY-RELATED FUNCTION.

THESE BREAKER ANOMALIES WERE PREVIOUSLY REPORTED TO THE NRC IN LER #97-024.

THE LICENSEE PLANS TO REPAIR AND TEST THESE BREAKERS (AND THE REMAINING 38 SAFETY-RELATED 480-VOLT BREAKERS) ON-SITE PRIOR TO INSTALLATION.

THE LICENSEE PLANS TO SUBMIT A WRITTEN PART 21 REPORT TO THE NRC.

THE LICENSEE WILL INFORM THE NRC RESIDENT INSPECTOR.
