This violation involved Calvert Cliffs Nuclear Power Plant (CCNPP) staff incorporating incorrect threshold values into its emergency action levels (EALs). Specifically, during the replacement of the Unit 2 main steam line radiation monitors, CCNPP's staff inaccurately calculated the associated EALs effluent threshold values for the Alert, Site Area Emergency, and General Emergency classifications. These thresholds were subsequently incorporated into Table R-1, "Effluent Monitor Classification Threshold" of the EALs. This calculation error could have resulted in an over-classification of an event, an unnecessary protective action recommendation, and could have caused offsite response organizations to implement unnecessary protective actions for the public. The finding is also associated with violation of NRC requirements specified in 10 CFR 50.54(q)(2), 10 CFR 50.47(b)(4), and 10 CFR 50, Appendix E. Immediate corrective actions taken to restore compliance included, upon discovery of the incorrect values included entering the issue into the corrective action program, implementing appropriate compensatory actions, and initiating corrective actions to revise the EAL table. |

This White finding involved the licensee's failure to develop and implement scheduled preventive maintenance for Agastat E7000 series time delay relays, as required by Technical Specification 5.4.1. Specifically, subsequent to the approval of Engineering Change Package No. ES200100067, issued in March 2001, the licensee did not replace the relays within the vendor recommended 10-year lifetime, nor establish a performance monitoring program. Consequently, on February 18, 2010, an Agastat E7000 series time delay relay that had a lifetime in excess of 10 years, used in the 2B emergency diesel generator (EDG) protective logic, timed out early and failed to support a demand fast start and run of the 2B EDG. This resulted in the EDG becoming inoperable with the resultant loss of alternating current to the 24 safeguards bus during the dual unit trip that occurred on February 18, 2010. |

The resulting plant condition was that no emergency diesel generator was immediately available to provide on-site back-up electrical power for a period more than 3 days. Additionally, the unavailability of the EDG resulted in a violation of Technical Specification 3.8.2, "AC Sources - Shutdown," which requires at least one EDG to be operable in Mode 5, and at least one EDG shall be restored immediately when it is determined that none is operable. |

The licensee determined the pump failed at the conclusion of its surveillance run on May 30, 2014, but this condition did not reveal itself until the pump failed to start on September 16, 2014. ORDER | 09/28/2015 | On September 28, 2015, the NRC issued a Confirmatory Order to Energy Northwest, to formalize commitments made as a result of an alternative dispute mediation session held on August 6, 2015. The commitments were made as part of a settlement agreement between Energy Northwest and the NRC regarding a violation that involved Nuclear Security Officers at Columbia Generating Station being willfully inattentive while on duty, which resulted in them not meeting the requirement to be available at all times inside the protected areas for their assigned response duties, contrary to 10 CFR 73.55(k)(5)(iii). In light of the significant corrective actions Energy Northwest had taken and subject to the satisfactory completion of the additional actions committed to take, as described in the Confirmatory Order, the NRC will not issue a Notice of Violation for the apparent violation. Those actions include, but are not limited to: (1) conducting a common cause evaluation, (2) revising its annual compliance and ethics computer-based training to address deliberate misconduct, (3) presenting at an industry forum to discuss the events that led to the confirmatory order, (4) conducting a targeted nuclear safety culture assessment, and (5) paying a civil penalty of $35,000. |

Failure to correct the condition adverse to quality resulted in inadequate procedures in that they contained steps that were inappropriate to the circumstances because they would not work as written to reposition the three motor-operated valves. |

Specifically, the licensee's inadequate procedural guidance for evaluating the suitability of parts used in safety related applications presented an opportunity in which the licensee failed to promptly identify a defective voltage regulator circuit board used in Emergency Diesel Generator (EDG) 2 prior to its installation. Following installation of the defective EDG 2 voltage regulator circuit board, the licensee failed to determine the cause of two high voltage conditions, and failed to take corrective action to preclude repetition. As a result, an additional high voltage condition occurred resulting in a failure of EDG 2. |
