03/12/2025 On March 12, 2025, the NRC issued a notice of violation to Mr. Mohammad Aljallad for a Severity Level III violation. Mr. Aljallad, a former Radiation Safety Officer for Hartford Hospital (Hartford) engaged in deliberate misconduct, a violation of Title 10 of the Code of Federal Regulations 30.10(a)(2). Specifically, he deliberately prepared and submitted a Hartford contamination spill report in which he documented inaccurate contamination survey results.
 
Miguel Marcial Amaro, IA-23-004 LETTER 05/22/2024 On May 22, 2024, the NRC issued a letter to Mr. Miguel Marcial Amaro, a former Mistras employee, stating he willfully falsified calibration certificates for non-destructive test equipment that was used at nuclear power plants. After considering a criminal conviction which includes a 5-year ban from participating or being employed in NRC licensed activities, the NRC has concluded no further action is warranted on this matter.
