Information Notice No. 83-34:   EVENT NOTIFICATION INFORMATION WORKSHEET 

Addressees: 

All nuclear power reactor facilities holding an operating license (OL) or 
construction permit (CP). 

Purpose: 

This information notice provides guidance to power reactor licensees 
regarding specific event notification information that should be transmitted
to the NRC Operations Center. 

Description of Circumstances: 

In order to facilitate rapid communication between licensee and the 
Operations Center in the early stages of an incident at an NRC licensed 
facility, NRC has developed an event information worksheet (published in 
NUREG-0845, Agency Procedures for the NRC Incident Response Plan). This 
worksheet is used by the NRC duty officers to obtain essential initial, 
event related information from the licensee. 

It is suggested that all licensees make copies of the worksheet available to
all their employees responsible for reporting event notification information
to the NRC. These individuals would normally use the Emergency Notifications
System (ENS) for reporting events but if unavailable may also use commercial
networks. 

No written response to this information notice is required. If you have any 
questions regarding this matter, please contact the Regional Administrator 
of the appropriate NRC Regional Office, or this office.
