NRC BULLETIN NO. 88-06:  ACTIONS TO BE TAKEN FOR THE TRANSPORTATION OF
                         MODEL NO. SPEC 2-T RADIOGRAPHIC EXPOSURE DEVICE


Addressees:

All NRC licensees authorized to manufacture, distribute, or operate 
radiographic exposure devices or source changers.

Purpose:

The purposes of this bulletin are: (1) to inform licensees of changes which 
have been made to NRC Certificate of Compliance No. 9056 for the Model No. 
SPEC 2-T radiographic device, and (2) to request licensees who possess or use 
the SPEC 2-T radiographic device to provide certain information to the 
appropriate NRC regional office.

On May 27, 1988 NRC issued Information Notice No. 88-33, "Recurrent Problems 
Involving the Model SPEC 2-T Radiographic Exposure Device."  This Bulletin is 
a followup to that Notice. 

Description of Circumstances:

As a result of the incident described in Notice No. 88-33, the NRC conducted 
an inspection at Source Production and Equipment Co., Kenner, Louisiana.  
Certain problems were identified in the manufacture of Model No. SPEC 2-T 
radiographic devices which included:

1.   The "as built" configuration of some of the devices does not conform to 
     the design approved by NRC, and

2.   Source Production and Equipment Co. failed to implement the quality as-
     surance program requirements specified in Subpart H of 10 CFR Part 71 and 
     in QA Program Approval No. 71-0102.

Actions Required:

All licensees in possession of the Model No. SPEC 2-T radiographic device 
shall be aware of the following conditions in revised Certificate of 
Compliance No. 9056 issued on June 8, 1988 for the Model No. SPEC 2-T 
radiographic device:

1.   For transportation in private carriage, the package must be either:

     a.   properly secured within a box attached to the vehicle for the 
          purpose of transporting or storing radiography devices, or

     b.   within a protective overpack which has a valid NRC Certificate of 
          Compliance that authorizes radiographic devices containing up to 200 
          curies of Iridium 192 as sealed sources in special form to be 
          shipped as contents of the overpack.

2.   For transportation by a common carrier, the package must be within a pro-
     tective overpack which has a valid Certificate of Compliance that 
     authorizes radiographic devices containing up to 200 curies of 
     Iridium 192 as sealed sources in special form to be shipped as contents 
     of the overpack.

3.   Promptly notify the appropriate NRC Regional Office in writing pursuant 
     to Section 182 of the Atomic Energy Act of 1954, as amended, and 
     10 CFR 71.39 of the following:

     a.   The serial numbers and purchase dates of Model No. SPEC 2-T 
          radiographic devices in the licensee's possession;

     b.   The method of enclosing or overpacking the devices when transported 
          in private or common carriage;

     c.   A statement that the licensee has registered with the NRC in ac-
          cordance with 10 CFR 71.12.

NRC licensees who do not take the above actions may be in violation of 
Commission requirements. 

This requirement for information was approved by the Office of Management and 
Budget under clearance number 3150-0011.
