BL74003A To All BWR and PWR With Current Operating License and Those Date: 4/26/74 Having An Anticipated Fuel DRO Bulletin #74-3A Load Date Prior to January 31, 1975 Gentlemen: The enclosed supplement to DRO Bulletin No. 3 "Failure of Structural or Seismic Support Bolts on Class I Components" is being sent to provide you with additional information relative to the omission of certain stiffeners and gussets in addition to defective welding reported at other facilities. This information may have applicability at your facility(ies). Action requested on your part is identified in Section B of the enclosed supplement. SUPPLEMENTAL INFORMATION RELATING TO RO BULLETIN NO. 74-3A Information was recently received that defective welding had been identified on steam generator support structures supplied to VEPCO, the North Anna, project, by Sun Shipyard Corporation of Chester, Pennsylvania. The Sun Shipyard Corporation also supplied similar items for the Public Service Electric & Gas Company, Salem 1 & 2, as well as the Consolidated Edison Company, Indian Point 3, projects. A. Description of Circumstances A special inspection of these items at the Salem site revealed not only areas of defective welding, but also omission of certain stiffeners and gussets shown on design drawings. The Consolidated Edison Company also noted similar type problems. The Sun Shipyard Corporation indicates that no similar components were provided other nuclear facilities. B. Action Requested In view of these particular type problems with seismic category 1 components as defined in Regulatory Guide 1.29, it is requested that: 1. The inspection program requested by Regulatory Operations Bulletin 74-3-"Failure of Structural or Seismic Support Bolts on Class 1 Components" include provisions that could detect problems of the type outlined above regardless of your supplier. . - 2 - As stated in Regulatory Operations Bulletin 74-3, RO inspectors will review your detailed written programs and procedures.
