Subject: DROPPED CONTROL ROD BLADE IN SPENT FUEL POOL
Discussion:
On July 23, 2003, at approximately 1:00 p.m., a control rod blade (CRB)
became detached from its storage location on the Unit 1 spent fuel pool
(SFP) wall and fell approximately 14 feet to the SFP floor. While
moving a CRB for processing and loading into a liner for eventual offsite
disposal, the cable being used became entangled with an adjacent CRB,
causing it to become detached from the SFP wall. The dropped CRB did
not contact any spent fuel and came to rest on the floor of the SFP at a
slight angle with the wall. It has been secured to a railing with a
tether and hook. There was no loss of SFP inventory nor any known damage
to the spent fuel in the pool. Each CRB is about 15 feet long and
weighs about 211 pounds (dry). The licensee was about 50% complete with
a campaign to process 30 CRBs and Incore Instruments into two radwaste
liners. All work has been stopped in Unit 1 until an events team
completes a root cause investigation. The licensee determined that this
event is not reportable. Courtesy notifications were made to the
resident inspectors and the Region based Unit 1 lead inspector.
