Information Notice 2001-08, "TREATMENT PLANNING SYSTEM ERRORS RESULT IN
DEATHS OF OVERSEAS RADIATION THERAPY PATIENTS," was issued on June 1,
2001.

This notice was issued to all medical licensees to alert them to a
significant event in Panama that may be applicable to any medical
facility using treatment planning software for therapeutic radiation
treatments.  On May 21, 2001, NRC was notified of an ongoing
investigation in Panama of 28 patients who received radiation therapy
doses up to 100 percent above what was prescribed.  Five patients were
reported to have died at that time, but the causes of their deaths are
under investigation.

Subject: MANAGEMENT CHANGES

Discussion:

Monticello announced the implementation of a new management structure.
Effective June 1, 2001, the following positions will report directly to
the Site Vice President, Mr. Jim Morris:

Plant Manager: Mr. Jeff Forbes, a former plant manager with Duke Energy
Corporation at Oconee Nuclear Station, relieved Mr. Byron Day
approximately 2 weeks ago. Mr. Day has been reassigned to Nuclear
Management Company (NMC) Headquarters in Hudson, Wisconsin, as Project
Director for the Integration and Coordination of the Fleet-wide
Management System for the NMC and will report to Mr. Charlie Schrock,
Senior Vice President of Operations, NMC.

Director of Engineering (new position): Mr. Dan Fadel, a former manager
with Ontario Hydro, reported onsite this past Monday. Before working for
Ontario Hydro, Mr. Fadel was Director of Engineering at the Palisades
Nuclear Station in Michigan.

Director of Site Support (new position): Mr. Mike Coyle, who reported to
the site approximately 3 weeks ago, is a retired naval officer. Most
recently, he was a manager with PECO at Clinton Station in Illinois. Mr.
Coyle will have responsibility for plant support groups such as training,
licensing, etc.
