Damage not Detected Due to Inadequate Assessment of Oil Analysis Results
and Selection of Pump Surveillance Points, dated May 11, 2001

notice to alert addressees that inadequate assessment of pump oil
analysis results, combined with surveillance testing which does not
monitor all relevant pump operating conditions, may allow severe pump
degradation to go undetected.
