Subject: Issuance of Regulatory Issue Summary (RIS) 99-03

NRC Regulatory Issue Summary 99-03 "Resolution of Generic Issue 145,
Actions to Reduce Common-Cause Failures" Dated October 13, 1999

INTENT

The U.S. Nuclear Regulatory Commission issued this regulatory issue
summary to notify nuclear power reactor licensees about the staff's
resolution of Generic Issue 145, "Actions to Reduce Common-Cause
Failures," and to communicate the broad insights that have been developed
from the staff's review of the common-cause failure events identified in
licensee event reports during the 15-year period between 1980 and 1995.

Subject: PALISADES SHUTDOWN FOR PLANNED REFUELING OUTAGE

Discussion:

The Palisades Nuclear Plant was taken off-line on October 15, 1999, at
11:24 p.m., for the scheduled 1999 refueling outage. In addition to the
refueling activities, other major maintenance activities that were
scheduled include: 1) Primary Coolant Pump P- 50D motor replacement; 2)
Primary Coolant Pump P-50A pump casing leak repairs and pump rebuild; 3)
replace shaft seals on three primary coolant pumps; and 4) replace both
low pressure turbine rotors on the main turbine generator.

Regional Action:

Resident inspectors observed portions of the reactor and plant shutdown
activities and will inspect various outage activities.
