SHEPHERD, MARK I IRRADIATOR Discussion: On April 23, 1999, the RSO for NIH informed Region I, Division of Nuclear Materials Safety, by telephone, that its J. L. Shepherd, Mark I irradiator had malfunctioned. NIH's license requires that they immediately stop use of the irradiator and notify the Commission by telephone if abnormal levels of radiation or any malfunction of the irradiator is detected. On April 21, 1999, NIH detected that the source had not fully retracted to the shielded position. The RSO indicated that there were no elevated radiation levels adjacent to the irradiator.
