AGREEMENT STATE REPORT - LOST RADIOACTIVE SOURCE

The following information was provided by the California Radiation Control Program (RHB) via email:

"On 7/12/23, Tesla, San Rafael facility contacted RHB to report a lost radioactive source. The lost item was a Po-210, 10 mCi source, Model P-2021-Z705, serial number A2LZ130 (air-gun), originally shipped to the facility by NRD, LLC on 5/8/2020, (based on this information, current activity is approximately 26 microcuries). The source was used to blow dust off of auto body panels and prevent static buildup prior to painting.

"The employees at the shop could not provide an estimate of when the last time the device was used and believe that the device may have been disposed of in the trash. Facility will be transitioning to an anti-static tool that does not utilize a radioactive source."

Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. AGREEMENT STATE REPORT - LOST TRITIUM EXIT SIGNS

The following information is a summary of information received from the California Department of Public Health, Radiologic Health Branch via email:

On July 12, 2023, Kaiser Medical Center of Southern California (the licensee) discovered that nine tritium exit signs were missing (Make: Shield, Model: SLX-60, Original activity: 10 Ci, tritium). The missing signs were exit signs at the licensee's parking garage. It has not been determined how the signs went missing after an engineering and security review.

AGREEMENT STATE REPORT - STUCK OPEN SHUTTER

The following information was received from the Texas Department of State Health Services (the Agency) via email:

"On July 12, 2023, the Agency was notified by the licensee's service company that during routine shutter checks, the shutter on a Vega Americas model SH-F2 [gauge] could not be shut. The gauge contains a 200 millicurie (original activity) Cs-137 source. Open is the normal operating position for the shutter. The licensee has made plans to repair the gauge in the next seven days. The service company stated there is no risk of radiation exposure to members of the general public or radiation workers due to this event. Additional information will be provided as it is received in accordance with SA-300."

EN Revision Text: ECCS POTENTIALLY INOPERABLE

The following information was provided by the licensee via email:

"At 0449 [EDT] on 7/12/2023, Millstone Unit 3 declared the 'B' train of the emergency core cooling system (ECCS) inoperable due to a degraded damper associated with the ventilation support system for the 'B' charging pump. At the time of this event, the 'A' train of service water was already inoperable due to planned maintenance on a breaker that would have prevented an 'A' service water valve powered from this breaker from closing on a safety signal. This configuration resulted in the possibility that the 'A' train of ECCS would not have been available to fulfill its design function under all postulated accident conditions. This event is being reported under 10 CFR 50.72(b)(3)(v)(B), '[any event or condition that at the time of discovery could have prevented the fulfillment of the safety function of structures or systems that are needed to: (B) remove residual heat].'

"Subsequently, the 'A' train of service water was restored to operable at 0548 on 7/12/2023. Repairs and investigation continue on the 'B' train ECCS damper. The NRC resident has been notified. This event did not impact Millstone Unit 2. There was no impact to the public."

The following information was provided by the licensee via email:

"The condition was reported to the NRC pursuant to 10 CFR 50.72(b)(3)(v)(B), via an 8-hour report as an event or condition that at the time of discovery could have prevented the fulfillment of the safety function of structures or systems that are needed to remove residual heat.

"A subsequent engineering review of the conditions that existed at the time determined that, based on area temperature response, any impact on ventilation flows into and out of the `B' charging pump cubicle did not generate an observable change in the temperature trend. Based on this, it is concluded with reasonable assurance that the functional requirement of the support system was maintained and the `B' charging pump would have continued to perform its safety function until the `A' train of service water was restored to operable and as a result safety function was not lost.

"Therefore, this condition is not reportable and NRC Event Number 56620 is being retracted.

"The basis for this conclusion has been provided to the NRC Resident Inspector."

"Tesla reported that one of their Polonium-210 static eliminators is missing. The device is a NRD model P-2021-Z705, s/n A2ME848. The device was shipped to Tesla on 2/23/21. The device has been through more than six half-lives so the activity at this point should be below 0.1 millicurie."

The source is sealed inside an air tool used to blow off dust from cars prior to painting. The company performed searches for the device, but was unable to locate the device. The device was discovered missing when the company was getting ready to return them to the manufacturer. The company stated that due to the remaining activity (purchased on March 30, 2021) and the design of the device, it does not pose an exposure risk to any individual."

AGREEMENT STATE REPORT - LOST RADIOACTIVE MATERIAL

The following information was provided by the Maryland Department of the Environment via email:

"On July 12, 2023, the Maryland Department of the Environment Radiological Health Program (MDE/RHP) was contacted via email from a Tesla employee who reported the loss of a device with NRD model number P-2021 Z705 from a Tesla Body and Paint facility located in Maryland.

"The lost device is manufactured by NRD, LLC with model number P-2021-Z705, serial number A2MF439. The device contains 10 mCi of Polonium-210. No reference date is associated with it, but the device was shipped to Tesla, Inc. on March 16, 2021. These devices are generally-licensed and sold to Tesla by NRD, LLC. Tesla, Inc. Maryland Site, NA-US-MD-Owings Mills-Body is located at 9430 Reisterstown Road, Owings Mills, Maryland 21117.

"On July 14, 2023, the Maryland Department of the Environment Radiological Health Program (MDE/RHP) investigation showed that the Po-210 radioactive material is used for alpha ionizer applications at the Tesla paint and body shop to stop dust from clinging to the body panels prior to painting. As per the company's procedure, when the device is no longer in use, the radioactive 'barrel' cartridges will be returned to NRD (or suitable disposer). The device had the 'gun' separated from the 'barrel' cartridge. The 'barrel' cartridge was returned to NRD as per procedure and the non-radioactive 'gun' part was lost or misplaced. In this process, the Po-210 source was not accounted for by NRD, LLC and reported as lost.

"This information is not conclusive as a lost source and needs to be further confirmed. Information on the current activity is required. MDE/RHP will finalize a reactive investigation."
