The following report was received from the state of California via email:
"On 08/06/21, California Office of Emergency Services (Cal OES) contacted Radiologic Health Branch (RHB) to report an incident involving loss of a Thermo Fisher Scientific, Niton X fill P 300 series XRF analyzer, Model XLP 300, S/N: 91800. The unit was resourced in May 2018 with 40 milliCi of Cd-109, with current activity of 6.7 milliCi. The XRF unit was taken from one of their employee's vehicle parked at his residence in San Jose, CA. A police report has been filed with the San Jose PD (report # is not available at this time). RHB will be following up with this investigation."

THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL
Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. The following report was received from the Utah Department of Environmental Quality, Division of Waste Management and Radiation Control (the Division) via email:
"The Division was notified of the medical event about 1155 MDT on August 10, 2021, by the licensee's Radiation Safety Officer. At about 0730 a male patient reported to Intermountain Medical Center to have a nuclear medicine scan conducted. The scheduled scan was to use 10 milliCi of Fluorodeoxyglucose (FDG) (F-18). The Technician administered the dosage to the patient and then realized he had administered the wrong dosage. The dosage administered was actually 104 milliCi of FDG which is much greater than the 10 milliCi that was prescribed. The licensee is in the process of notifying the referring physician and the patient of the medical event. The licensee is aware of the 15 day requirement for a written report.
"The Division will investigate this matter and update the record upon completion of the investigation."
A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient.

The following report was received from the state of Tennessee via email:
"Power loss site wide at Energy Solutions. Power was restarted by the utility. However, the power at the Radioactive Materials Solutions (RMS) building did not return. As a result, the ventilation system at the RMS building was not functional. The back up generator failed to function under load of ventilation fans. Currently, the RMS building operations have ceased due to loss of power. An area monitor is in place and showing no signs of elevated activity from air monitoring inside of the building. The contractor was contacted to reestablish power to site. Security patrols have been established to monitor quantities of concern. A follow-up report will be submitted to include corrective actions from the licensee."

"At 0024 EDT on 8/18/21, an unusual event was declared (EAL HU 4.1) due to receipt of multiple fire alarms and halon discharge in the cable tunnel. At 0036, the fire brigade verified no signs of fire. Unit 1 remained at 100 percent power and stable."
The area is currently being ventilated. Unit 2 was not affected by this event.
The licensee has notified State and local authorities and the NRC Resident Inspector.
R1 Public Affairs (Screnci) was notified.

"RCW 49.17(1)(a) requires an employer report 'within 24 hours.that 10 or more of their employees at the workplace or worksite in this state have tested positive for the infectious or contagious disease that is the subject of the public health emergency, must report the positive tests to the department in a form prescribed by the department.' This is the new Health Emergency Labor Standards Act - HELSA. ESSB 5115.SL
"On 8/17/2021 the Framatome Horn Rapids Road site hit that threshold of 10 COVID positive cases. These cases do not appear to be worker-to-worker transmissions of the virus.
"This notification is being made under the concurrent reporting requirement of 10CFR70 Appendix A concurrent reporting."

"At 1036 CDT on 8/18/2021, Wolf Creek experienced a reactor trip due to low level in B Steam Generator. Auxiliary feedwater system actuated as designed. All systems actuated as expected. Decay heat is currently being removed by the auxiliary feedwater system. The NRC Senior Resident Inspector has been informed.
"All control rods fully inserted, and offsite power remained available."
