"RCW 49.17(1)(a) requires an employer report 'within 24 hours.that 10 or more of their employees at the workplace or worksite in this state have tested positive for the infectious or contagious disease that is the subject of the public health emergency, must report the positive tests to the department in a form prescribed by the department.' This is the new Health Emergency Labor Standards Act - HELSA. ESSB 5115.SL

"On 8/17/2021 the Framatome Horn Rapids Road site hit that threshold of 10 COVID positive cases. These cases do not appear to be worker-to-worker transmissions of the virus.

"This notification is being made under the concurrent reporting requirement of 10CFR70 Appendix A concurrent reporting."

EN Revision Text: MEDICAL TREATMENT WITH CONTAMINATION

"On August 17, 2021, at approximately 0230 [EDT], a Westinghouse employee was washing piping over a container of nitric acid in the Conversion Decontamination Area. The piping fell into the container of nitric acid and employee reached into the container to retrieve the piping and received nitric acid burns to hands and left wrist. Appropriate treatment for exposure to nitric acid was provided by on-site medical response staff. With an abundance of caution, after review with on-site medical, the employee was transported to an off site medical facility [Prisma Hospital]. Per procedure the employee's hands and arm were wrapped in plastic, and the employee was transported to an off site medical facility accompanied by plant health physics personnel for evaluation.

"Contamination was detected on the exposed area of the employee's skin during Health Physics surveys. Direct survey results were 1500 dpm/100 cm squared alpha for the left hand and 900 dpm/100 cm squared alpha for the right hand. All smear results of the exposed area were below clean area limits (<200 dpm/100 cm squared). Contamination surveys were performed in the ambulance and at the hospital and all results were below clean area limits indicating no spread of contamination during care for the employee. All potentially contaminated materials associated with the issue were collected and returned to the [Commercial Fuel Fabrication Facility] CFFF for disposal. Operator was provided with over the counter medication and released.

"The task that the employee was performing required a chemical suit, chemical gloves, fresh air bubble hood, chemical boots, and required taping the gloves to the sleeve of the acid suit jacket with chemical tape. After inspection of employee's personal protective equipment, it was noted that chemical tape was not applied to the gloves which enabled the nitric acid solution to enter the left glove and acid suit sleeve when employee reached into the nitric acid container."

EN Revision Text: AGREEMENT STATE REPORT - RADIOACTIVE MATERIAL LEFT AT RECYCLING CENTER

The following was received from the Maryland Department of Environment Radiological Health Program via email:

"On August 17, 2021, at about 1215 EDT, the Maryland Department of the Environment Radiological Health Program (MDE/RHP) was contacted via telephone by the Operations Manager of the Montgomery County Shady Grove Transfer Station and Recycling Center located at 16101 Frederick Rd, Derwood, Maryland 20855 that a "B17 Bomber", radioactive material was thrown into a scrap metal bin. The MDE/RHP responded the same day and investigated the "B17 Bomber", which was later identified as Sextant Bubble Type (with Altitude Averaging Device) AN-5851-1, Part number 3014-1-B and Serial Number AF-42-0676, Contract number AC-26968 and manufactured by Bendix Aviation Corporation navigation instrument which contain Radium - 226 source with estimated nominal activities of 2 microcuries.

"The Sextant Bubble Type navigation device was dropped by unidentified person(s) at an unknown date and time and was discovered by the Montgomery County Shady Grove Transfer Station and Recycling Center staff when screened for radiation on August 17, 2021. The device was isolated and stored at the temporary hazardous materials storage in the facility by the Operations Manager. The device was later transferred to the local radioactive waste management company, the RSO, Inc. for disposal on August 20, 2021.

"MDE/RHP will finalize a reactive investigation."

The following was received by the Arizona Department of Health Services (the Department) via email:

"The Department received notification on August 26, 2021, from the licensee, that during an inspection on August 17, 2021, to close the source down for a day, the source handle was broken and the shutter was not able to be closed. The licensee taped off the area and called the vendor to come service the gauge. The gauge was removed from service on August 20, 2021. The gauge was a Berthold Density Gauge, Model #LB7440-D-CR, Serial Number B0514, with an activity of 150 mCi of Cs-137. The Department has requested additional information and continues to investigate the event.

"Additional information will be provided as it is received in accordance with SA-300."
