| - SHIPMENT OF 8 Ir-192 SOURCES (866 CURIES TOTAL) FROM BURLINGTON, MA, |
| OVERDUE IN MEXICO - |

| At 1500 EDT on 04/21/99, Ken Traegde of the Massachusetts Department of |
| Public Health Radiation Control Program (RCP) located in Boston, |
| Massachusetts, reported that on 04/16/99, AEA Technology QSA, Inc., located |
| in Burlington, Massachusetts, shipped a Model-855 source changer loaded with |
| eight Iridium-192 sources (totaling 866 curies) via Federal Express to |
| Vicont S.A. De C.V. located in Toluca, Mexico. The undamaged package had a |
| transport index of 1.4 (normal readings of 1.4 mR/hour at 1 meter and 55 |
| mR/hour at the surface) at the time of shipment. |
| |
| The shipment was scheduled to arrive in Mexico on 04/17/99. As of 1400 EDT |
| on 04/21/99, the recipient (Vicont S.A. De C.V.) had received the shipping |
| papers but not the container. The shipper and Federal Express have launched |
| a full search for the package. The Federal Express Hazardous Material |
| Investigation Team has conducted a physical search of Express Depots in |
| Boston, Memphis, Los Angeles, and Mexico. |

| A conference call was conducted at 1230 on 04/23/99 to update the status of |
| this event. The following NRC offices participated (among others): IRO |
| (Himes), NMSS (Brach, Combs, and Cook), Region 1 (Lloyd), Region 2 (Collins |
| and Woodruff), Region 4 (Andrews), OPA (Hayden), OIP (Lieberman), OSP |
| (Lohaus), and EDO (Raddatz). In addition, the following agencies |
| participated: Department of Transportation (Rick Boyle), Massachusetts |
| Department of Public Health RCP (Ken Traegde), Federal Express (Scott Mugno, |
| Pat Oppenheimer, and Roy Parker), State of Tennessee (Alan Grewe), Tennessee |
| Division of Radiation Health (Eddy Nanney), AEA Technology QSA, Inc. (Eric |

| During this conference call, representatives of Federal Express reported |
| that they are confident that the shipment of eight Iridium-192 sources left |
| Boston. They issued an 'all-points' search order, faxed a description to |
| all 'logical' locations, and conducted repeat physical searches in Memphis, |
| Boston, and Mexico, all without locating the shipment to date. (The Los |
| Angeles depot was not searched as had been reported on 04/21/99.) |
| Massachusetts and Tennessee health authorities conducted radiological |
| surveys in Boston and Memphis on 04/22/99 with negative results. |
| |
| Rick Boyle of the Department of Transportation notified the competent |
| authority in Mexico, and Federal Express requested a detailed search of the |
| Customs facilities in Mexico City. |
| |
| The next conference call is scheduled for 1230 EDT on Wednesday, 04/28/99. |

| At 0619 EDT on 04/26/99, Federal Express confirmed that the serial number on |
| a Model-855, Type-B package found at the Stansted Airport (located outside |
| of London, England) matched the serial number for the missing package |
| containing the eight Iridium-192 sources. The Massachusetts Department of |
| Public Health RCP was informed of this discovery by AEA Technology QSA, |
| Inc., at 0933 EDT on 04/26/99. The package was reported to be intact and |
| undisturbed. The carrier (Federal Express) contacted the shipper (AEA |
| Technology QSA, Inc.), and the shipper requested that the package be |
| forwarded to its intended destination. Accordingly, the package will be |
| shipped from Stansted Airport to Toluca, Mexico, via Memphis, Tennessee. |
| Federal Express informed the Massachusetts Department of Public Health RCP |
| that the package is expected to arrive in Memphis, Tennessee, by tomorrow |
| morning (04/27/99). |

| The Massachusetts Department of Public Health RCP notified NRC Region 1 |
| (Duncan White). The NRC Operations Officer notified the R1DO (Anderson); |

| - ONE CONTROL ROD INSERTED INTO THE REACTOR VESSEL WITH THE FUEL OFF LOADED |
| - |
| |
| While all fuel was removed from the Brunswick Unit 2 reactor vessel for a |
| scheduled refueling outage, an inadvertent reactor protection system |
| actuation resulted in the insertion of one control rod (#50-31). The |
| invalid scram signal was due to maintenance already underway on the "2A" DC |
| switchboard (removed from service under clearance, half scram signal present |
| on RPS "A" channel due to de-energization of reactor protection system |
| instrumentation) and an inadvertent high level trip of average power range |
| monitor B due to working on local power range monitoring instrumentation |
| underneath the reactor vessel (half scram signal on the RPS "B" channel). |
| The one control rod which moved had been withdrawn in preparation (or |
| placing it under clearance) for control rod drive replacement. Seven other |
| control rods were completely withdrawn and under clearance to support |
| inspection work inside the reactor vessel. These rods did not move during |
| this event, due to being under clearance. This event has no safety |
| significance. There was no fuel inside the reactor vessel at the time of |
| this event. |

| A SOURCE RACK FAILED TO RETURN TO THE POOL AT AN IRRADIATOR FACILITY |
| |
| A source rack failed to return to the shielded position in the pool at |
| Sterigenic's irradiator facility in Gurnee, IL. The licensee investigated |
| the problem and determined that a bolt was getting hung up in the guide |
| cable and mechanically binding. They were able to lower and unload the |
| source rack and make repairs. The source rack has been reloaded and the |
| facility is back in operation. No injuries or over exposures occurred as a |
| result of this event. |

| TEN EMERGENCY SIRENS INOPERABLE |
| |
| "On 4/30/99 at 1332, Brunswick experienced a loss of power to ten offsite |
| sirens. Eight of the sirens are in Brunswick County and the other two are |
| located in the Carolina Beach area of New Hanover County. The site |
| experienced winds in excess of 50 mph when the sirens were determined to be |
| lost. |
| |
| "The function of the offsite siren system is to provide early warning to the |
| public in the event of an accident. Emergency Management personnel of the |
| affected counties have been notified to perform backup route alerting |
| communication for these areas as necessary. Based on this interim measure, |
| the safety significance is considered minimal. |
| |
| "Repairs are currently in progress. Transmission crews are on the scene in |
| the Carolina Beach area. The two sirens in Carolina Beach have been |
| repaired." |
| |
| The NRC resident inspector has been informed of this notification by the |
| licensee. |

| A fiber optic phone cable has been cut offsite that defeats the ability of |
| the normal phone system for the Emergency Operations Facility (EOF) and |
| Joint News Center. Additionally, the Radiological Emergency Communications |
| System for county and state communications has been lost to the EOF. As a |
| backup, cell phones and the normal offsite radio network facility have been |
| tested to the EOF. Additionally, the site pager system can be used to |
| contact Emergency Planning personnel to initiate communications. The repair |
| is estimated to take 8 hours. |

| AT 0916 ON 05/01/99, JERRY EAKINS, FLORIDA BUREAU OF RADIATION CONTROL, |
| REPORTED THAT A TROXLER MOISTURE DENSITY GAUGE WAS STOLEN FROM DELTA |
| CONSTRUCTION COMPANY, MIAMI, FLORIDA AT 0130. THE GAUGE, MODEL #3430, |
| SERIAL #28191, CONTAINING 8 mCi OF Cs-137 AND 40 mCi OF Am-241/Be, WAS |
| STOLEN FROM THE BACK OF A PICKUP TRUCK. THE LOCKS ON THE TRUCK AND THE |
| STORAGE CONTAINER WERE BROKEN AND ONLY THE GAUGE WAS STOLEN. THE COMPANY |
| OWNER, STEVE McCOY, NOTIFIED THE MIAMI POLICE (CASE #121-1051Y) AND PLANS TO |
| PLACE AN AD IN THE MIAMI NEWSPAPER OFFERING A REWARD FOR THE RETURN OF THE |
| GAUGE. |
