questionnaire to provide data for a study we are performing on the reliability
of standby diesel generators.

Due to a clerical oversight, parts 6 and 7 of item Q on page 18 were omitted
from the questionnaire sent to you.  Please utilize the enclosed page 17 (the
unchanged reverse side) and corrected page 18 in preparing your response.
