1. DISCOVERY DATE: 6/18/99
2. DESCRIPTION OF DEFICIENCY:
A safety related DB-25 Breaker Mechanism, procured from Westinghouse, did not pass degradation checks when drawn from stock to be installed into Bus15/03A. The Breaker Mechanism has holes drilled that do not line-up, causing the pin above the toggle lever to be out of alignment. Also, the right rear quarter section of the tripper pan is bent upward, giving us unsatisfactory tripper bar gaps.

4. DATE WHEN EVALUATION WILL BE COMPLETED:
A purchase requisition has been sent to Westinghouse for a failure mode assessment. It is estimated that the evaluation will be completed by December 1, 1999.
