Part 21 Report - 1997-211

ACCESSION #: 9703130317 C O V E R Facsimile S H E E T To: Directors Office of Inspection and Enforcement Fax#: (301) 816-5151 Subject: Over-stress of aux. hoists on Whiting Cranes Date: March 12, 1997 Pages: 6, Including this cover shoot. COMMENTS: Recently, we at Whiting have become aware of a condition of an over-stressed auxiliary hoist on one of our cranes. Based on this, we have reviewed a series of cranes and are in the process of sending notices to affected customers. Attached is a sample letter and a list of the 11 utility companies that we have notified of this condition. The authorization of loads up to 40% of rated capacity, as described in this fax, is a conservative generalization and will be addressed on a case-by- case basis with each customer. Separate specific instructions by Engineering or a fax will authorize loads as high as 60% of rated capacity for a specific application. The cranes as designed meet the current requirements of AISC, but violate the requirements of CMAA. We are authorizing operation at a reduced capacity which will bring operation into conformance with both requirements. We do not believe this to be reportable under 10 CFR 21, but we have notified these companies so that they may do further evaluations. If further information is required, I can be contacted at (708) 331-4000, extension 4020. Sincerely, Mark J. This is to inform you of a concern related to the continued operation of crane(s) 10044. This notification is limited, and only customers receiving this notice are required to take action. An analysis of the auxiliary hoist on a crane similar to the referenced crane(s) has revealed some overstressed conditions. These over-stressed points arc not likely to result in a failure, but are being reported so that further evaluations and corrections can be made. The main hoist is not affected by this condition and can continue to be used with out restriction. As an interim measure we recommend taking the auxiliary hoist out of service rather then derating because variations in your design require further evaluation be done before use. If the above provisions are followed, the crane can continue to be used without additional concern. Whiting Corporation, a Delaware Corporation, has no direct responsibility for the design of equipment sold by Whiting Corporation, an Illinois Corporation, prior to an asset purchase in 1994. Based on current information, it does not appear that this condition is reportable under 10 CFR 21. However, we are passing this information on to all affected utilities for their evaluation. Until the time repairs can be made, you must either discontinue use or derate the capacity of the auxiliary hoist to 40% of the original rated capacity on the referenced cranes. Whiting Corporation and our sister company Whiting Services Inc. are willing to assist in repair of these trollies by providing engineering, repair parts bills of material, and field services Page 1 of 2 Mark Kwasny - Whiting Corporation RE: Aux.
