DUQUESNE LIGHT-BEAVER VALLEY IDENTIFIED SEVERAL DEFECTIVE HEX    
CAPSCREWS SUPPLIED BY CARDINAL INDUSTRIAL PRODUCTS DURING        
ACCEPTANCE TESTING. ADDITIONAL TESTING BY CARDINAL CONFIRMED THAT
SEVERAL PRODUCTION LOTS WERE IMPROPERLY HEAT TREATED AND DID NOT 
DEVELOP FULL STRENGTH.                                           
                                                                 
CARDINAL HAS IMPOUNDED THE DEFECTIVE FASTENERS IN THEIR BONDED   
NONCONFORMANCE AREA, AND CLOSED THEIR HEAT TREAT FACILITY PENDING
FURTHER INVESTIGATION.   USE OF DEFECTIVE FASTENERS COULD CREATE 
A SAFETY HAZARD. THE FOLLOWING LICENSEES HAVE BEEN IDENTIFIED AS 
RECEIVING POTENTIALLY DEFECTIVE FASTENERS:  DUQUESNE LIGHT;      
GEORGIA POWER (VOGTLE); VERMONT YANKEE; ILLINOIS POWER; T.V.A.;  
COMMONWEALTH EDISON; HANFORD; AND ANO 1. OTHERS MAY HAVE
RECEIVED
DEFECTIVE FASTENERS FROM SUPPLIERS OR HARDWARE DISTRIBUTORS.     
                                                                 
THE DEFECTIVE CAP SCREWS WERE 3/8 x 4 A-193 B7 IDENTIFIED BY HEAD
MARKINGS "B7, C, TS9, S4" FROM LOT NUMBERS 12133, 12134, AND
