THE LICENSEE REPORTED THAT THE BUTTERFLY VALVE MANUFACTURED BY
FISHER CONTROLS WAS SUPPLIED AS AN AIR-TO-OPEN VALVE INSTEAD OF
AN AIR-TO-CLOSE VALVE. THIS VALVE IS A BASIC COMPONENT IN THE
SERVICE WATER OUTLET FOR CONTAINMENT RECIRCULATION FAN COOLERS, A
SAFETY HAZARD COULD BE CREATED BY SUCH A DEFECT, WHICH WOULD
CAUSE THE LOSS OF SERVICE WATER TO THE FAN COOLERS. CORRECTIVE
ACTION WAS COMPLETED WHEN THEY ALTERED THE ACTUATOR LEVER WHICH
RESULTED IN THE VALVE FUNCTIONING CORRECTLY. THIS VALVE WAS
PURCHASED AS A REPLACEMENT FOR A SPECIFIC SERIAL NUMBERED ITEM.
THE ORIGINAL ENGINEERING REQUIREMENTS FOR THIS VALVE STIPULATED
AIR-TO-CLOSE OPERATION.
