On February 3, 1995, Problem Evaluation Report (PER) WBPER950078 was initiated to document an apparent failure to follow the requirements of Site Standard Practice (SSP) 3.04, "Corrective Action Program." The problem identified in the PER was that some contractor individuals may not have been documenting conditions adverse to quality through any of the WBN Administrative Control Programs identified in SSP-3.04. 10 CFR 50.55(e)(1)(i) requires that deficiencies must be evaluated for reportability within 60 days of discovery. Additional time is required to determine whether the undocumented deficiencies in WBPER950078 could have created a substantial safety hazard, if left uncorrected. This action will be completed by June 3, 1995. The Enclosure identifies the commitment made in this submittal.
