Re:  Crystal River Unit 3, Request for Corrective Action No. RCA 0387

In accordance with 10CFR21. para. 21.21 (c)(4), Notification, we
respectfully submit the following written report:

5. DESCRIPTION OF DEFICIENCY: VSL Corporation furnished commercial grade
/non-safety related studs, nuts and washers for use on safety-related
Crystal River #3 5th year post-tension surveillance program.  The studs,
nuts and washers were purchased by FPC from VSL as safety-related
(Reference Attachment Q ) to FPC Purchase Order L810079D.  During a joint
Utility Audit of VSL Corporation that was performed on December 6-8,
1994, the audit team discovered that VSL had purchased the studs, nuts
aid washers as non-safety related from an unapproved supplier that had
not been audited or evaluated by VSL.  VSL Purchase Order # 226-0013-7242
to Lord & Sons for these items, did not impose the requirements of FPC
Purchase Order Attachment Q (10CFR21).  The items ordered were 3/4" - 10
X 4 1/2" A193-B 7 ATR Studs with matching nuts and washers.

FPC recommends that the following conditions be evaluated by VSL during
the development of your corrective action response: 1) VSL to review for
potential 10CFR21 reportability.  2) Develop and submit for FPC review a
commercial grade dedication procedure.  3) Provide a positive corrective
action plan that identifies the cause, corrective action to correct this
condition and actions to prevent recurrence.

                         CORRECTIVE ACTION REPLY

9. CAUSE AND EXTENT OF PROBLEM VSL normally furnishes this material for
the same application, as commercial grade, non-safety related, for all
other nuclear customers.  For Florida Power however, these should have
been first dedicated for safety-related prior to supply.  This
discrepancy was unintentional, presents no safety hazard and is an
isolated instance.  To comply with requirements, additional bolts will be
supplied for testing under the program for commercial grade dedication.

10. CORRECTION ACTION TO PREVENT RECURRENCE The commercial grade
dedication procedure is attached and will be used during future
procurements of this nature.  The individual involved has been informed
of the situation and understands the requirements.  Future personnel will
also be indoctrinated through Q.A. in the processing of orders to assure
the commercial grade procedure is correctly implemented.  The NRC was
notified of this event as required in 10CFR21 (copy attached).
