Discussion:

On November 13, 2005, while in a refueling outage, Crystal River Unit 3 identified two broken reactor internal baffle plate bolts while investigating fuel grid strap wear concerns. The failed bolts are 7/16" stainless steel baffle-to-baffle bolts used to join individual baffle plates, vice baffle-to-former plate bolts which hold the baffle plates to the horizontal former plates within the core barrel. While the baffle-to-baffle bolts play a role in maintaining the integrity of the baffle assembly, the baffle-to-former bolts are the critical components essentials to support the vendor's structural analysis of the baffle assembly.

The licensee is currently defueling the reactor to facilitate follow on actions. Framatome ANP is assisting the licensee in their determination of follow on actions which will include an analysis of the required bolting for continued operation, and ultrasonic testing of selected baffle-to-former plate bolts.
