Discussion:

As discussed in MR 2004-0039 from Region III (ADAMS Accession No. ML043150408), Leonard Clewett, the Operation Director at Three Mile Island (TMI), accepted a position as the Plant Manager for Nuclear Management Company's Prairie Island Station. Pending selection of his replacement, AmerGen named Randy Campbell, the Assistant Operations Manager, as the acting Operations Director. Mr. Campbell's selection was effective on Friday, November 12.

Failure of Harris Feedwater Isolation Valve During Refueling Outage - Update

Discussion:

Morning report 2-2004-0016 was issued on November 10, 2004, for a valve stem failure on the Shearon Harris C Main Feedwater (MFW) isolation valve. Subsequently, the licensee inspected all three MFW isolation valves and found damage to the seat rings on all three valves. The licensee determined that the seat ring damage was due to overtravel of the double wedge gates, on the opening stroke, which allowed the gates to travel beyond the seat rings. This resulted in contact between the gates and the top of the seat rings on the closing stroke. The overtravel of the gates was caused by insufficiently sized spacers to limit the stem travel.

The damaged seat rings were replaced on all three valves and new spacers were installed to limit the opening stroke. The licensee performed a foreign object search and retrieval (FOSAR) inspection of the MFW piping between the isolation valves and the vertical risers inside of containment. Foreign objects with material characteristics identical to the seat rings were found in one MFW header. However, further review indicated that any foreign material of sufficient size to severely damage a steam generator tube would not be able to pass from the feed system to the steam generator.
