Subject: SEVERED PLUGGED TUBE DISCOVERED IN STEAM GENERATOR

Discussion:

On April 15, 2002, the licensee informed the NRC that they had discovered
a severed plugged tube in the 1B steam generator during outage
inspections.  Tube 78-124 was severed at the lower tube sheet.  The
licensee found the severed tube when an adjacent tube's plug was pulled
for repair and wear scars were detected on it by eddy current
examination.  The licensee removed a section of the severed tube for
metallurgical examination and root cause analysis.

Regional Action:

Under the guidance of NRC Management Directive 8.3, Region II initiated a
Special Inspection on April 17, 2002.  The location of the severed tube
indicated that the failure mechanism was different than a recent severed
tube at Three Mile Island 1, as described in NRC Information Notice
2002-02, Recent Experience With Plugged Steam Generator Tubes.  The
specific issue of concern to be addressed by the inspection is to obtain
information so that the generic implications of the tube failure can be
assessed.
