MR Number: 2-02-0006 Harris 1 Date: 04/15/02 Raleigh,North Carolina Dockets: 50-400 PWR/W-3-LP Subject: RESULTS OF SPECIAL INSPECTION AT HARRIS Reportable Event Number: N/A Discussion: On April 9, 2002, Region II initiated a Special Inspection at the Shearon Harris Nuclear Power Plant following the licensee's identification of gas accumulation in the charging/safety injection pump (CSIP) common suction header. On April 12, 2002, Region II completed the Special Inspection and an exit meeting was held with the licensee. The inspection reviewed the facts surrounding this gas accumulation event. The charging safety injection pumps are currently operable. Followup ultrasonic monitoring has not identified any additional gas accumulation. The preliminary root cause appears to be degassing of reactor coolant in the residual heat removal (RHR) loop piping without adequate venting. Specifically, following RHR venting prior to startup after the recent outage, the unit was cooled down from Mode 3 and RHR depressurized to repair a leaking conoseal. This apparently allowed degassing to occur. Subsequent venting of the RHR was not performed to remove the gas in the RHR system. Through a series of system manipulations the gas apparently moved to the high point of the CSIP common suction line. The inspectors also concluded that the Harris safety injection system piping does not have sufficent high point vents to ensure the suction piping is full of water. The licensee is performing a past operability evaluation of the CSIP's. The evaluation will be reviewed by the staff when complete. Regional Action: None. Contact: BRIAN R. BONSER (404)562-4560
