MR Number: 2-01-0015 Turkey Point 3 4 Date: 08/20/01 Miami,Florida Dockets: 50-250,50-251 PWR/W-3-LP,PWR/W-3-LP Subject: MANUAL REACTOR TRIP AND SUBSEQUENT RESTART Discussion: On August 15, 2001, at 4:49 a.m., Unit 3 was manually tripped from 15 percent power due to a reduction in condenser vacuum. Two intermediate range nuclear instrumentation detectors and one individual rod position indicator (all rods were inserted fully) did not function as expected following the reactor trip. The unit was placed in Hot Standby for repair activities. The licensee determined that the reduction in condenser vacuum was the result of leakage through holes in two tubes in the main condenser. The holes were caused by loose flashing material inside the condenser (sheet metal used to protect expansion joints and bellows) impacting the tubes. The affected tubes were identified and repaired. The licensee replaced both intermediate range detectors and is evaluating the cause of the failures. The rod position indicator issue was attributed to a control panel indicator problem and was corrected. Unit 3 returned to power operation on August 20, 2001. Regional Action: For information only.
