Koshy, REXB The Beaver Valley Unit 1 licensee reported that a steam-driven auxiliary feedwater pump failed to start from breakage of a cap screw on the collar of the hydraulic balancing drum. The head of the screw became lodged between the stuffing box extension and the collar, preventing the pump from starting. The licensee determined the cause of the failure to be a manufacturing defect, which propagated due to tensile stress and hydrogen absorption at the tip of the crack. The licensee stated that only one of the four cap screws installed in the pump contained material defects, though the licensee identified deficiencies with respect to the purchase order related to material composition and hardness for all four screws. Cap screws were initially provided as part of a new pump rotating assembly purchased in 1990 from Ingersoll-Dresser Pump Company (now Flowserve Corporation). In 1991, the licensee changed the pump internals to stainless steel, based on an Ingersoll-Rand Part 21 notification (see Part 21 91-09-0, dated September 19, 1991, accession number 9110100007, and NRC Information Notice 88-87, "Pump Wear and Foreign Objects in Plant Piping Systems," November 16, 1988). Koshy Email: txk@nrc.gov 301-415-1176 _ REGION IV MORNING REPORT PAGE 2 MARCH 7, 2001 Licensee/Facility: Notification: Nebraska Public Power District MR Number: 4-01-0002 Cooper 1 Date: 03/07/01 Brownville,Nebraska Resident Inspectors Dockets: 50-298 BWR/GE-4 Subject: UPDATE - LEVEL CONTROL PROBLEMS FOLLOWING REACTOR SCRAM AT COOPER (Event 37805) Discussion: This morning report is being updated to correct technical information related to the availability of the reactor feedwater pumps and to supply additional information about the licensee response. Following the automatic trip of reactor feedwater Pump B, the pump would not restart when operators attempted to return it to service. When operators started reactor feedwater Pump A, it exhibited higher than normal vibration. The operators returned reactor feedwater Pump A to standby and initiated the reactor core isolation cooling pump to supply water to the reactor coolant system. The licensee is conducting a human performance review to gain a better understanding of licensed operator performance during the event response. Regional Action: The senior resident inspector was onsite for most of the event. Following the stabilization of reactor parameters, the resident inspectors provided continuous onsite coverage until the reactor reached cold shutdown. The NRC senior risk analysts have evaluated the risk and concluded that no special inspection efforts are warranted. The resident inspectors will monitor licensee actions.
