Subject: Start of 2000 Refueling Outage
Discussion:
On April 22, 2000, Kewaunee commenced a refueling outage with the opening
of the main generator output breaker. Major planned work activities
include modifications to the safety injection system, replacement of both
diesel generators' coolers, replacement of the spent fuel pool heat
exchanger tube bundle, and eddy current testing of the steam generators.

Subject: UPDATE TO 4/18/00 MORNING REPORT -- EQUIPMENT QUALIFICATION
DEFICIENCIES WITH OKONITE TAPED SPLICES
Discussion:
On April 22, licensee management issued a stop work order on the
application of 3M tape on environmentally qualified splices until
resolution of discrepancies associated with the 3M qualification test
report. The 3M tape splices were being used to replace existing Okonite
splices. Identified discrepancies in the qualification test report
included the installation methodology and the adequacy of the electrical
testing of the representative splice.
As of April 24, the licensee was continuing efforts to identify the scope
of the use of taped splices. Licensee personnel inspecting terminal
boxes in the reactor building over the weekend identified an additional
26 EQ splices with Okonite tape treatments. Licensee management has
stated that all identified Okonite tape on environmentally qualified
splices will be replaced.
The licensee ended their refueling outage on April 21 stating that all
refueling work was complete and declared a forced outage for the purpose
of repair and resolution of the issues related to taped splices at the
facility.
Regional Action:
NRC personnel are evaluating the licensee's actions to correct EQ taped
splice deficiencies.
