Petrone As discussed in Morning Report H-99-0094 dated November 1, 1999, the Crystal River licensee issued Part 21 Report No. 99-43-0 on October 6, 1999. In that report the licensee reported finding damaged insulation on the same conductor in each of six samples of three-conductor cable (BICC Brand Rex Company power cable #1108582, 1 kV, 3/C, #2/0, Class B, 90C, XLPE Insulated, Black Jacket). The licensee was installing this cable, procured as safety-grade and environmentally qualified cable, in a nonsafety-related application. The licensee found the damage on removing the outer jacket for termination. The licensee learned from the vendor, Brand Rex, that during cable fabrication, while combining the three conductors before closing, the damaged conductor fell off the cabling unit pulley due to loss of tension, causing the insulation to be scraped. This morning report provides additional information obtained from conversations with a licensee representative on November 2, 1999 and a vendor representative on November 3, 1999. This additional information confirmed that the cable had been manufactured as a special order for Crystal River. The vendor's representative stated that they had reviewed their records and confirmed that only four reels of cable had been manufactured using the special equipment and procedures and that all four of those reels had been delivered to Crystal River. No other cables had been manufactured using the same special equipment and procedures and therefore they were certain that they had not supplied similar defective cable to any other nuclear licensee. The vendor has since modified the equipment and instituted new testing procedures to preclude similar defects if these cables are manufactured in the future. Based on this new information it appears that this particular defect is not a generic concern. _ REGION I MORNING REPORT PAGE 2 NOVEMBER 3, 1999 Licensee/Facility: Notification: Connecticut Yankee Atomic Power Co. MR Number: 1-99-0042 Haddam Neck 1 Date: 11/02/99 Hartford,Connecticut Dockets: 50-213 PWR/W-4-LP Subject: TRANSPORT OF HADDAM NECK STEAM DOMES Discussion: On November 2, 1999, Connecticut Yankee Atomic Power Company started the transport of major reactor components from the Haddam Neck Nuclear Power Plant as part of the decommissioning process. The first of four steam domes was transported by road over a four-hour period on the afternoon of November 2, 1999, to a rail yard in Portland, Connecticut. On November 3, 1999, the steam dome was transferred to a rail car for trasnport to the GTS-Duratek processing facility in Tennessee. Each cylindrical steam dome weighs about 77 tons and is approximately 20 feet long and 12 feet in diameter. The domes are transported as over-weight and over-size loads. The level of radioactivity in the steam domes, which is similar to the turbine rotors shipped earlier this year, is minimal and did not require special packaging or labeling for radioactive material. The licensee provided notification to local elected officials, police and fire departments, and school superintendents and issued a press release to alert the public of traffic delays, because of the slow speed of the carrier. The State of Connecticut has also reviewed the transportation plan. Regional Action: NRC inspected the package and conducted independent measurements prior to the package leaving the site. As expected, no radiation above background was detected. In addition, the inspector accompanined the shipment to the rail yard to observe the lifting of the heavy load onto the rail car.
