Subject: ELECTRICAL INDUSTRIAL ACCIDENT

Discussion:

On July 16, 1999, at approximately 1030 hours, an electrician performing
routine preventative maintenance on a 480 volt motor controller received
an electrical burn. The individual was transported initially to the local
hospital in Pike County and then transported to Ohio State Burn Center in
Columbus, Ohio. As of July 19, 1999, at 0800 hours, the individual
remains in intensive care with burns to the hands and face.

The certificatee stopped preventive maintenance activities on motor
controllers until an event investigation is completed.

Regional Action:

The resident staff is monitoring the certificatee investigation.
