MR Number: 3-99-0044 Fermi 2 Date: 05/18/99 Newport,Michigan SRI via telecon Dockets: 50-341 BWR/GE-4 Subject: FERMI UNIT 2 MANUALLY SCRAMMED FOLLOWING TRIP OF THE "A" REACTOR RECIRCULATION PUMP Discussion: The Fermi 2 nuclear plant was manually shut down by operators following the unexpected trip of a reactor recirculation pump. On May 18, 1999, while an electrical maintenance individual was replacing a field brush on the "A" motor generator (MG) set, an electrical wire contacted the center slip ring on the motor end of the MG set. The resulting ground fault caused a generator lockout relay to trip which tripped the "A" reactor recirculation pump. Reactor power remained at 97 percent as core flow dropped to approximately 40 percent because of the pump trip. This placed the reactor in the scram region of the Technical Specification power to flow map. In accordance with procedures, the operators placed the mode switch to the "shutdown" position which scrammed the reactor. All rods inserted as expected, no emergency core cooling system equipment actuated and no safety relief valves lifted in response to the event. All systems responded as expected.
