MR Number: 3-99-0036 Monticello 1 Date: 05/03/99 Monticello,Minnesota E-mail from Resident Inspectors Dockets: 50-263 BWR/GE-3 Subject: Reactor Startup from Forced Outage Discussion: On May 1, 1999, the licensee completed an unplanned outage and returned the unit to 100 percent power. The outage began on April 22, 1999, when the reactor scrammed due to low reactor water level which was caused by a malfunction in the feedwater control system. The licensee repaired the feedwater control system and performed other minor outage related maintenance prior to the startup. Regional Action: The resident inspectors observed portions of the outage maintenance activities and portions of the reactor startup. Contact: R. MR Number: 3-99-0037 Quad Cities 1 Date: 05/03/99 Cordova,Illinois E-Mail Dockets: 50-254 BWR/GE-3 Subject: Unit 1 Startup Following Outage To Repair Jet Pump Riser Crack Discussion: Unit 1 startup commenced on Thursday, April 29, 1999, following an April 10 planned shutdown to install a brace made necessary due to a reactor recirculation jet pump riser crack. The Unit 1 reactor was critical on April 30, and the generator was synchronized to the electrical grid on May 1, 1999. The crack on the jet pump 7 and 8 riser pipe was identified during the Unit 1 refueling outage which ended December 5, 1998. Operations were limited during the subsequent period to prevent additional cracking before repairs could be made. Other outage activities included noble metal injection for chemistry control, performance of 18-month interval surveillance logic tests, and containment piping overpressure modifications. MR Number: 3-99-0038 Duane Arnold 1 Date: 05/03/99 Palo,Iowa SRI via LAN Dockets: 50-331 BWR/GE-4 Subject: PLANT STARTUP FOLLOWING REPAIRS TO MAIN GENERATOR FIELD GROUND Discussion: On April 29, 1999, at 7:42 p.m. (CDT), the licensee commenced a reactor startup following a shutdown to identify and repair an intermittent main generator ground. The reactor was critical at 11:22 p.m. and the main turbine was connected to the grid at 7:19 p.m. on April 30. The licensee identified a torn copper "leaf" used to connect the positive main terminal studs to the main generator field. The main terminal studs were last removed during an outage in 1988 to support main generator preventive maintenance work. The main terminal studs were over-torqued when reinstalled which led to the torn copper leaf. Through years of operation the torn copper pealed away from the main terminal stud due to centrifugal force and caused the intermittent electrical grounds, which initially started on January 1, 1999. No other major work activities were conducted during the 11-day outage. Regional Action: The resident inspectors observed portions of the plant startup and power ascension activities.
