Information Notice 99-11, "INCIDENTS INVOLVING THE USE OF RADIOACTIVE
IODINE-131," was issued on April 16, 1999.

This notice was issued to all medical licensees to alert them to recent
incidents that resulted in unintended radiation doses to humans, as a
result of the administration of radioactive iodine.  These cases
illustrate the need for communication and coordination between the
authorized user and the referring physician.  They also illustrate the
importance of verifying correct dosages and patient identity before
administration.
