NEW ISSUE. On December 7, 1998, during a quarterly surveillance, a scram
discharge volume (SDV) drain valve failed to close at Vermont Yankee. It
was refurbished, tested, and returned to service on December 10, 1998. On
December 11, 1998, during accelerated testing of other SDV drain valves,
two more were found that did not meet stroke time requirements of the
inservice testing program (see Licensee Event Report 50-271/98-025-01).
The licensee reports that the root cause of these failures appears to be
an inadequate actuator sizing calculation for the design conditions
specified in the licensee's procurement specification. The valve and
actuator were supplied by BW/IP International, Inc., Valve Division, of
Williamsport PA. The licensee replaced all the valve actuators in this
system with larger actuators and is addressing questions regarding the
vendor's quality assurance program.

Subject: Change in Status of Braidwood Unit 1
Discussion:
On April 5, at approximately 2:00 a.m., operators at ComEds Braidwood
Station commenced a Unit 1 shutdown following a failure of a rupture disc
on the Unit 1 heater drain tank. The turbine generator breaker was opened
at 9:30 am, April 5, 1999, and the reactor was shutdown at 11:00 am,
April 5, 1999. The shutdown was observed by the resident inspectors. An
investigation into the cause of the rupture disc was initiated. On April
8, after relocating the rupture disc to a different section of the
piping, a startup of the unit was commenced. Plant return to service is
expected the evening of April 8, 1999.
