MR Number: 3-99-0026 La Salle 1 Date: 03/25/99 Marseilles,Illinois RI via Telephone Dockets: 50-373 BWR/GE-5 Subject: UNIT 1 FUEL ASSEMBLY LEAK Discussion: On March 25, 1999, the licensee identified an increase in the offgas system pre- treat radiation release rate. Levels increased from approximately 50 mrem per hour before the leak to approximately 80 to 90 mrem per hour after the leak was identified. Radiation levels subsequently steadied out at approximately 70 mrem per hour. The dose-equivalent iodine increased from approximately 8.08 x 10-5 to 1.39 x 10-2 microcuries per gram. The Technical Specification limit was 0.2 microcuries per gram. The licensee entered the failed fuel procedure, which required increased offgas and reactor coolant system sampling to monitor for further degradation. On April 2, 1999, the licensee commenced a power reduction to perform flux suppression testing of the reactor core in order to identify the location of the leaking fuel assembly. The licensee successfully identified the location of the leaking assembly and suppressed the flux around the bundle by fully inserting three adjacent control rods.
